In order to better understand how caring is impacting on you, we would encourage you to complete an Adult Carer Assessment and Support Plan (ACASP). You can download the form by clicking the link below. Once you have completed the form, it should be emailed to acasp@carerswd.org or, if you would prefer, print it and send it to us at:

Who qualifies for an Adult Carer’s Assessment and Support Plan?
Anyone who is providing care for a friend or relative or is intending to provide care, regardless of how many hours this is for. The person that you care for must live in the West Dunbartonshire area. However, you do not need to be in receipt of Carers’ Allowance and you do not need to live with the person that you care for.

Why is it important?
The purpose of the ACASP is to allow you the opportunity to think about and discuss how caring is impacting you, and what help you may need with caring, including maintaining your own health and balancing other commitments such as work. The information you provide can help identify what support we may be able to offer you.
